Job summary

RHA Health Services in Wyomissing, PA is seeking a Blended Case Manager (Regular) to advocate for clients, connect them with community resources and develop individualized service plans. You will collaborate with care teams to ensure comprehensive support across behavioral health and medical needs.

Requirements include a Bachelor’s degree in a human services field (or an Associate degree with 12 credits and two years of relevant experience) and a valid driver’s license.

Qualifications

• Bachelor's degree in human services related field (sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education) OR Associates Degree with 12 College Credits relevant to position and two years' experience in mental health/human service field.

Responsibilities

• Develops service plans, treatment plans and individual crisis plans based on client strengths and needs.
• Prepares and completes documentation including assessments, admissions, medication records, progress notes and crisis plans within required timeframes.
• Assists client in gaining funding authorization for needed services and entitlements.
• Participates in agency investigations.
• Performs other related duties as assigned.
