Job summary

Evolve Therapy Services, LLC seeks a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Newark, OH. You will support the Physical Therapist, implement treatment plans, and participate in resident screenings and staff education to promote high-quality care.

This full-time role requires an Associate's degree in physical therapy and an active PTA license in the state, with ongoing professional development and CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine intervention needs.
• Assist in evaluation and in developing target goals and treatment plans.
• Deliver individual and group treatments focusing on strength, mobility, balance, and ROM.
• Adapt techniques and activities based on resident performance and PT guidance.
• Monitor responses and adjust plans with the Physical Therapist.
• Assist in scheduling and coordinating resident therapies with the team.
• Ensure timely, compliant documentation in CMS/system records.
