Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility partner in Newark, OH. This full-time role supports the Physical Therapist in delivering high-quality therapy services.

You will screen residents, assist in evaluations and treatment planning, and help implement individual and group therapies while documenting in NetHealth EMR. APTA-aligned ongoing education and licensure maintenance are required.

Formación

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsabilidades

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying target goals and treatment plan.
• Conduct both individual and group treatment as assigned and outlined in the treatment plan.
• Assist in selecting PT techniques/media and sequence activities with adaptation as indicated.
• Monitor residents’ response to intervention and consult with the PT if modification is needed.
• Establish and modify resident schedules with the PT and other team members.
• Adhere to department and resident schedule; modify as needed for treatment.
• Train resident, family and appropriate staff in equipment use.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Report to work on time and coordinate schedule for productivity.
• Assist with Quality Assurance program as required.
• Complete complaint and medically necessary documentation in NetHealth EMR.
