Job summary

Evolve Therapy Services in Newark, OH is seeking a dedicated Physical Therapy Assistant to support an in-house Skilled Nursing Facility. Under the supervision of a Physical Therapist, you will provide targeted therapy, screen residents, and help develop treatment plans to improve mobility and functional independence.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.
• CMS/NetHealth EMR documentation compliance.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluating residents and in developing targeted treatment plans.
• Assist the Physical Therapist in identifying goals and timelines for treatment.
• Conduct individual and group treatments as outlined in the care plan.
• Train residents and family in the use of equipment and home exercises.
• Maintain accurate, timely documentation and participate in QA activities.
