Job summary

South Carolina Department of Social Services is seeking a CWS Case Manager Transition Support Services to perform case management and planning for youth aging out of foster care. You will engage with teens and young adults using trauma-informed approaches, ensuring safety, permanency and well-being.

The role requires ongoing documentation in CAPSS, coordination with legal to ensure compliance, and regular training. A valid driver’s license and ability to transport youth are required.

Qualifications

• Requires a Bachelor's Degree in Social Work, Behavioral Science or Social Science.
• A Bachelor's in another field with one year related experience qualifies.
• A Master's Degree or higher may substitute for required experience.
• Child welfare experience is preferred.

Responsibilities

• Provide case management and planning for Extension of Foster Care (EFC) within DSS policy and GPS Practice Model.
• Maintain monthly face-to-face and frequent virtual contact; document activities in CAPSS system.

Education

Bachelor's Degree in Social Work, Behavioral Science, or Social Science

Bachelor's Degree in any other field with 1 year related experience

Master's Degree or higher may substitute for required experience

Prefer child welfare experience
