Job summary

The South Carolina Department of Social Services is seeking a Case Manager for the Extension of Foster Care (EFC) program. You will provide case management for foster young adults, engage with empathy, document activities in CAPSS, and support transition planning toward self-sufficiency.

You will complete CANS assessments, coordinate services, and collaborate with the legal team to ensure program compliance and successful outcomes for participants.

Qualifications

• Master's Degree and 1 year of professional experience in social services programs, correctional, education, business administration, general administrative, management, or other relevant program experience.
• Bachelor's Degree and 3 years of professional experience in social services programs, correctional, education, business administration, general administrative, management, or other relevant program experience.
• Prefer child welfare experience.

Responsibilities

• Perform case management for current and former foster young adults in the Extension of Foster Care (EFC) Program.
• Engage with empathy, maintain monthly face-to-face or frequent virtual contact, and document in CAPSS.
• Complete assessments (CANS) and develop individualized transition plans with young adults.
