Job summary

South Carolina Department of Social Services is hiring a CWS Case Manager TSS to support current and former foster young adults (18-21) in the Extension of Foster Care program. You will provide case management, service navigation, and trauma-informed support in collaboration with the Transition Services and Support Division.

You will maintain ongoing engagement, document activities in CAPSS, and participate in court processes as required.

Qualifications

• Master’s Degree and one year of professional experience in social services programs, correctional, education, business administration, general administrative, management, or other relevant program experience.
• Bachelor’s Degree and three years of professional experience in social services programs, correctional, education, business administration, general administrative, management, or other relevant program experience.
• Prefer child welfare experience.

Responsibilities

• Performs case management for current and former foster young adults in the EFC program.
• Maintains engagement with empathy and respect; provides transition support and service referrals.
• Completes and updates assessment tools (CANS) and develops individualized transition plans with the young adult.
• Provides Service Navigation by identifying resources and assisting with referrals or applications.
• Engages families and kin to support safety planning and placement.
• Prepares for Young Adult Transitional Teaming (YATT) and contributes to permanency planning.
• Maintains compliance with DSS policies and participates in court proceedings as needed.
