Necco seeks a Foster Care Case Manager to work with children, foster families, and schools to achieve permanency. You will develop and monitor treatment plans, coordinate assessments, and connect families with behavioral health, education, and community resources.

Qualifications

• 21 years of age.
• LSW or LPC or Bachelor's degree in a human services-related field (OH, KY, GA).
• Bachelor's degree with field experience of 2 years (WV).
• Bachelor's degree or Master's degree from an accredited four-year college or university, with at least 7 hours in behavioral sciences such as social work, psychology, and sociology.
• One year of casework experience can be substituted for the 7 hours.
• Minimum of 2 years experience working with children with emotional or behavioral problems.
• Valid Driver's License.
• Organizational, and Written/Oral Communications skills.
• Must have the skills necessary to navigate several software systems (MS Office, Databases, EHRs, Dashboards, etc.).
• Successful completion of all required Criminal Background checks and drug screenings per state and company regulations.
• Ability to adhere to scheduled and unplanned deadlines.
• Willingness to travel and attend training.
• Successful completion of all required training pertaining to job.

Responsabilités

• Participate in referral matching for children and families, advocating for best home match.
• Develop Initial Treatment Plan and Comprehensive Treatment Plan for each child.
• Facilitate or ensure completion of assessments (Biopsychosocial, Independent Living, Diagnostic, etc.).
• Refer foster child to applicable support services (behavioral health, independent living, etc.).
• Ensure initial legal documentation is obtained prior to placement or service delivery.
• Develop and implement Individualized Treatment Plans (ITP) and monitor progress.
• Train and support foster parents and children; ensure CPT adherence.
• Coordinate with placement agencies and community resources; liaise as needed.
• Ensure compliance with state licensure and accreditation requirements; maintain confidentiality.
• Deliver services across in-home and community-based settings; travel between locations as essential.
