Good Shepherd Services in the Bronx, NY is seeking a dynamic Case Planner to support timely permanency planning for children and youth in foster care. This role requires in-office work with field visits and occasional hybrid scheduling based on program needs.

Reporting to the Foster Care Supervisor, you will assess safety, coordinate visits, connect families with resources, and document case progress in accordance with agency standards.

Qualifications

• Bachelor’s Degree in related field required.
• MSW or related experience preferred.
• Ability to establish working relationships with clients.
• Ability to engage parents.
• Strong communication and group facilitation skills.
• Some evenings may be required.
• Commitment to strength-based youth development.

Responsabilités

• Assess and monitor safety and risk.
• Establish contact with families and foster families.
• Conduct required casework contacts with child, parent, foster parent and providers.
• Visit family and child to gather social history and assess progress.
• Coordinate and supervise visits between family and child.
• Collaborate with team for permanency planning.
• Locate resources and make referrals for families.
• Monitor child’s adjustment and educational/behavioral performance.
• Keep case records up to date and complete all agency documentation.
• Make court appearances and testify at permanency hearings.
• Attend Family Team Conferences and required meetings.
• Use Sanctuary Model tools and keep records.
• Submit progress notes promptly.
• Support foster parents and involve them in goals.
