Zusammenfassung

Good Shepherd Services is actively seeking a dynamic Case Planner to advance permanency planning for children and youth in foster care. You will work closely with the Foster Care Supervisor and program teams in Bronx, NY, contributing to safety planning, service coordination, and court documentation.

The role requires collaboration with families, schools, and service providers, with in-office hours and field visits. A strong commitment to trauma-informed practice is essential.

Qualifikationen

• Bachelor’s Degree in related field required.
• MSW or related experience preferred.
• Must be able to establish working relationships with clients in need of counseling and supportive services.
• Must be able to effectively engage parents.
• Ability to apply theory of change.
• Willingness to facilitate groups with parents and foster parents.
• Ability to work cooperatively with supervisor(s), KEEP, PTC, YDSC consultants, ACS and other team members.
• Excellent written and verbal skills necessary.
• Some evenings may be required.
• Commitment to work from a strength based and/or youth development perspective.

Aufgaben

• Assess and monitor safety and risk for children in foster care.
• Establish contact with families and foster families.
• Conduct required casework contacts with child, parent, foster parent and collateral contacts.
• Visit family and child to gather social history and assess family strengths and concerns.
• Coordinate and supervise all visits between family and child.
