Job summary

Good Shepherd Services, Ltd seeks a dynamic Case Planner to advance permanency planning for children in foster care. The role reports to the Foster Care Supervisor and is based at 7 West Burnside Avenue, Bronx, NY with hybrid in-office/field responsibilities.

The ideal candidate will have a related degree (MSW preferred), strong communication and group facilitation skills, and the ability to engage families and coordinate visits. Great teamwork and commitment to youth development are essential.

Qualifications

• Bachelor’s Degree in related field required.
• MSW or at least one year related experience preferred.
• Must be able to establish working relationships with clients.
• Must be able to effectively engage parents.
• Ability to apply theory of change.
• Willingness to facilitate groups with parents and foster parents.
• Ability to work cooperatively with supervisors, KEEP, PTC, YDSC consultants, ACS and team members.
• Excellent written and verbal skills.
• Some evenings may be required.
• Commitment to work from a strength-based and youth development perspective.

Responsibilities

• Assess and monitor safety and risk.
• Coordinate visits between family and child along with separate sibling visits.
• Conduct required casework contacts with the child, parent, foster parent and collateral contacts.
• Visit family and child to gather social history and assess concerns.
• Share responsibilities with agency team for permanency planning.
• Locate resources and make referrals to aid the family and child.
• Evaluate child’s adjustment in foster home and monitor educational and behavioral performance.
• Monitor foster parents and make recommendations regarding future use of the home.
• Adherence to manualsKEEP/PTC/YDSC.
• Make efficient use of training materials and participate in SBC training.
