MercyFirst is seeking a Case Planner to support children in foster care, partnering with Socio-Therapists to aid families toward permanency. The role requires travel across NYC, Long Island, Staten Island, Upstate NY and possibly NJ, with some weekends and overnight visits.

Qualifications

Bachelor's in social work or related field required.
1 year of related work experience with children/families.
Knowledge of child development stages and family dynamics.
Excellent verbal and written communication skills.
Proficiency in Microsoft Word and Outlook.

Responsibilities

Provide intensive support to children, therapeutic foster parents and birth families.
Conduct regular casework sessions with each client per mandates.
Conduct regular home visits to therapeutic foster homes.
Maintain contact with birth parents and foster parents for problem solving.
Document contacts and activities in State system and agency records.
