Job summary

TemPositions Group of Companies is seeking a dedicated Case Planner for a foster care/child welfare program in New York. The role involves managing cases for children, foster families, and biological families with aim of achieving permanent placement promptly.

Responsibilities include assessment, planning, advocacy, and collaboration with schools and providers, plus court involvement and thorough documentation. Flexibility for evenings/weekends and travel is expected.

Qualifications

• Bachelor’s degree in a human services field (BA/BSW) required.
• Minimum 2 years of human services experience.
• Spanish bilingual a plus.

Responsibilities

• Assess each child and family’s needs and create/implement treatment and service plans.
• Provide hands-on case management: advocacy, problem-solving, referrals, and life-skills support.
• Maintain regular contact with children, foster parents, and biological families, including facilitating visits and supporting relationships.
• Coordinate with schools, service providers, and community resources to track progress and address barriers.
• Prepare for and attend court hearings, including testifying when needed.
• Plan for discharge/permanency by connecting families with community services.
• Complete extensive documentation (reports, plans, progress notes) and ensure compliance with agency and ACS requirements.
• Monitor children’s well-being (education, health, recreation, independent living skills).
• Participate in meetings, trainings, and supervision.
