Holy Family Institute is seeking a Case Manager for the Journey of Hope LTFC program in Moon Township, PA. The role involves coordinating intake assessments, ISPs, family reunification, and discharge while ensuring thorough documentation and compliance with ORR/DPW standards.

The position requires 3–5 years of human services experience, clearances (Act 33/34, FBI), and a valid PA driver’s license. Expect supervision, ongoing trainings, and adherence to HIPAA protocols.

Qualifications

• 3–5 years of progressive experience in human services.
• Act 33/34 and FBI clearances required.
• PA driver’s license required.
• Must be at least 21 years old.
• Ability to lift or push a minimum of 20 lbs. and up to 50 lbs. with assistance.

Responsabilités

• Coordinate case management services, including intake assessment, ISP development, family reunification, and discharge.
• Ensure proper documentation in each child’s case file.
• Submit incident, ChildLine, and CPS reports within mandated timelines.
• Maintain HIPAA and ORR/DPW compliance and confidentiality.
• Engage in supervision, trainings (40+ hours annually) and ensure staff meet job qualifications.
