Qualifications

Responsibilities

MISSION AND NEED:

GENERAL STATEMENT OF DUTIES

The Case Manager is responsible for coordinating case management services. The Case Manager's tasks shall include development of intake assessment, individualized care plan, family reunification, permanency planning, post-18 planning, transition to adulthood, independent living support, and discharge planning. The Case Manager shall also ensure that all services provided to each child are properly documented in each child's case file. Final approval for hire will be done by the Project Officer of the Office of Refugee and Resettlement which is a sub-agency of the Department of Health and Human Services of the federal government.

RESPONSIBILITIES:

• Works with a multifunctional team to maintain well-being and safety of minors and to ensure that services are provided.
• Maintains cultural sensitivity to work effectively with diverse populations of minors and coworkers.
• Compiles, prepares, maintains, and submits documentation in accordance with ORR and DPW guidelines.
• Identifies a sponsor and assesses suitability based on requirements for safe release.
• Document progress that families and sponsors make toward risk reduction, achievement of service goals and positive case outcomes.
• Completes assessments within allocated timeframe to maintain compliance.
• Coordinates release of minor and facilitates referrals as needed.
• Participates in weekly meetings with ORR representatives to provide feedback and progress on cases.
• Participates in team, supervisory, and other program meetings as needed.
• Utilizes the assistance of Case Aides to achieve program goals.
• A willingness to travel when needed to support case-related activities, youth transitions, reunification, discharge, transfers, placements, or other program-related needs. Able to work holidays when needed, as well as participate in an on-call rotation.
• Models and assists in the implementation of the positive behavior management techniques, such as HART Model and Trauma-Informed Care
• Ensure confidentiality in all program operations and service population.
• Uphold professional and ethical standards of conduct outlined in the DS employee handbook
• Promptly reports civil or criminal convictions, charges, arrests, investigations, or adjudications that may arise since staff member'’s last background investigation. This includes but is not limited to sexual abuse in a prison, jail, holding facility, community confinement facility, juvenile facility, other institution (as defined in 42 U.S.C. 1997), or care provider facility; conviction of engaging or attempting to engage in sexual activity facilitated by force, overt or implied threats of force, or coercion, or if the victim did not consent or was unable to consent or refuse; or who was civilly or administratively adjudicated to have engaged in such activity.
• Completes other duties as assigned by supervisor

REQUIREMENTS:

• Bachelor's degree in behavioral sciences, human services or social services fields including psychology, sociology, social work or counseling.
• Must possess exceptional time management and detail-orientation skills.
• Child welfare and/or case management experience is strongly encouraged.
• Candidate must be fluent in both English and Spanish (read, write, and speak).
• Excellent written and verbal communication skills.
• Must be proficient in Microsoft Word, the Internet, and e-mail.
• Act 33/34 and FBI Clearances Required.
• Must be at least 21 years of age
• Must have a valid driver's license and be able to drive a car or van. Ability to remain calm and composed under stress
• Ability to safely and successfully perform essential job functions consistent with the ADA, FMLA, and other federal, state and local standards
