Job summary

Hoyleton Youth & Family Services seeks a dedicated case manager to deliver clinical and case management services for foster children and families. You will monitor placements, develop permanency plans, and coordinate birth parent services to support reunification, guardianship, or adoption as appropriate.

Qualifications

• Understanding the issues of separation and loss and trauma in the child.
• Ability to help parents improve parenting capacities and develop stable permanency plans.
• Respect for the child’s cultural background, heritage, and identity in service delivery.

Responsibilities

• Deliver clinical and case management services to foster children and families.
• Monitor foster placements through home visits monthly or per service plan/DCFS contract.
• Develop and implement service plans to achieve permanency and evaluate progress.
• Coordinate for birth parent services, ensure visitation and sibling contact as required.
