Job summary

Holy Family Institute is a long-established Catholic organization in the Pittsburgh region seeking a Case Manager to coordinate case management services for minors, including intake assessments, care planning, family reunification, and discharge planning.

The role requires a Bachelor's degree in behavioral sciences or related fields, fluency in English and Spanish, strong time management, and the ability to travel and work holidays as needed. Equal opportunity employer.

Qualifications

• Bachelor's degree in behavioral sciences, human services or social services fields including psychology, sociology, social work or counseling.
• Exceptional time management and detail orientation skills.
• Child welfare and/or case management experience is encouraged.
• Fluency in English and Spanish (read, write, speak).
• Excellent written and verbal communication skills.
• Proficient with Microsoft Word, Internet and e-mail.
• Clearances (ACT 33/34 and FBI) required.
• Must be at least 21 years old and hold a valid driver's license.

Responsibilities

• Coordinate case management services and develop intake assessments and individualized care plans.
• Support family reunification, permanency planning, and transition to adulthood.
• Document services accurately in each child’s file and ensure compliance with ORR/DPW guidelines.
• Collaborate with a multifunctional team to ensure wellbeing and safety of minors.
• Travel as needed; participate in on-call rotations and holiday coverage.
• Promote trauma-informed care and positive behavior management techniques.
• Maintain confidentiality and uphold professional standards.
