Children’s Hope Alliance is seeking a dedicated Foster Care Case Manager to support children, empower foster families, and help build stable, nurturing environments where youth can thrive. You will coordinate services, lead Child and Family Team Meetings, coach foster parents, conduct in-home visits, and ensure documentation and licensing standards are met, while collaborating

Qualifications

• Bachelor's degree in Psychology, Social Work, or a related field.
• At least 2 years of experience working with children, families, or in a related human services role.
• Qualified Professional (QP) designation is required.
• Ability to respond to urgent situations and participate in on-call rotation.

Responsibilities

• Coordinate services, treatment planning, and resources to support each child’s unique needs.
• Facilitate Child and Family Team Meetings and contribute to admission, treatment, and discharge planning.
• Provide ongoing coaching, training, and hands-on support to foster parents.
• Maintain accurate documentation and monitor licensing standards and case records.
