FamilyForward, a nonprofit focused on supporting vulnerable children through therapeutic and education services, is seeking a Foster Care Case Manager. You will decide on client service plans across medical, educational, social, and emotional needs, ensuring access to necessary resources for quality care.
You will handle on-call emergencies, coordinate permanency goals, prepare detailed records, and lead Family Support Team meetings.

In addition to our work with foster care and adoption, FamilyForward also provides specialized services to prevent and treat abuse and neglect: trauma assessment and psychological evaluation, therapy, coaching and education, therapeutic preschool, and care for children with developmental disabilities.

This position requires sustained periods of work on the computer, including writing/typing. This position requires frequent communication with others, in-person and via telephone/virtual conferencing. The employee will be asked to move around rooms or within a space. The employee may occasionally be asked to lift and/or move up to 25 pounds, or greater with assistance. The employee must have transportation to all Agency sites, including external locations where events are held. This position requires frequent travel to client visits or other meetings outside of Agency sites. This position occasionally requires evening and/or weekend work.
