Job summary

NECCO (ED Necco & Associates Inc.) in Kentucky is seeking a Foster Care Case Manager to support children and foster families across office, school, and community settings. You will help develop and monitor individualized treatment plans, coordinate services, and ensure compliance with state regulations to achieve permanency.

You'll partner with schools, health providers, and social service agencies to meet education goals, deliver trainings to foster parents, and maintain thorough documentation.

Qualifications

• 21 years of age
• Current LSW or LPC or Bachelor’s degree in human services (OH, KY, GA)
• Bachelor’s degree with field experience of 2 years (WV)
• Bachelor’s or Master’s from a 4-year college; at least 7 hours in behavioral sciences
• Minimum of 2 years of casework experience with children
• Valid Driver’s License
• Organizational and Written/Oral Communications skills
• Ability to navigate software such as MS Office, databases, EHRs, dashboards

Responsibilities

• Participate in placement/referral matching for children and families
• Develop and monitor Initial and Comprehensive Treatment Plans
• Complete assessments (Biopsychosocial, Independent Living, Diagnostic)
• Refer foster child to behavioral health, independent living, etc.
• Ensure initial legal documentation is obtained
• Develop and implement safety plans during intake
• Monitor and review medication logs for compliance
• Deliver services across in-home and community settings and travel as needed
