Qualifications

• Minimum age 21.
• Initial treatment planning and comprehensive treatment planning.
• Assessment documentation (Biopsychosocial, Independent Living, Diagnostic, etc.).
• Compliance with state licensure regulations for contact with foster children.
• Willingness to travel for home visits and trainings.
• Excellent written and verbal communication.

Responsibilities

• Participate in referral matching to ensure best home match.
• Develop Initial and Comprehensive Treatment Plans.
• Facilitate Assessments (Biopsychosocial, etc.).
• Refer to support services (behavioral health, independent living).
• Ensure initial legal documentation is obtained before placement.
