Job summary

Necco is seeking a Foster Care Case Manager to work with children and foster families across office, community, school, and home settings. The role focuses on building permanency by equipping families with needed support and resources, while adhering to state licensure and accreditation requirements.

The candidate will engage in intake, treatment planning, continuous coordination with care teams, and community resources. Travel between locations as part of essential duties is expected.

Qualifications

• Must be at least 21 years old.
• Bachelor's degree in a human services-related field (OH, KY, GA).
• LSW or LPC license or pursuing licensure.
• Two years field experience with related populations (WV).
• Valid driver’s license and auto insurance coverage.

Responsibilities

• Participate in referral matching for children and families.
• Develop Initial and Comprehensive Treatment Plans.
• Coordinate assessments (Biopsychosocial, Diagnostic, etc.).
• Refer to support services and ensure plan adherence.
• Maintain confidentiality and comply with regulations.
• Serve as liaison with placement agencies and community resources.
• Attend service meetings with stakeholders (schools, clinicians, etc.).
• Ensure children receive medical and therapy services as per plan.
• Provide training and ongoing support to foster families and children.
• Monitor documentation for licensure/compliance and CAP tasks.
• Complete home visits and coordinate discharge planning.
