Job summary

Necco in Ohio seeks a Foster Care Case Manager to work with children and foster families across office, community, school, and home settings, helping children reach goals and achieve permanency.

You will coordinate treatment plans, assess needs, and connect families with behavioral health, education, and community resources while maintaining confidentiality and complying with state regulations.

Qualifications

• At least 21 years of age.
• Current LSW or LPC or Bachelor's degree in a human services-related field.
• Bachelor's degree with field experience or equivalent required.
• At least 7 hours in behavioral sciences (social work, psychology, sociology) or one year of casework can substitute.
• Minimum 2 years' experience working with children with emotional/behavioral problems.
• Valid Driver’s License and required auto insurance.
• Ability to use MS Office, databases, EHRs and dashboards.

Responsibilities

• Participate in referral matching and advocate for best home matches.
• Develop and monitor Initial and Comprehensive Treatment Plans.
• Coordinate assessments (Biopsychosocial, Independent Living, Diagnostic).
• Refer youth to behavioral health, independent living, and related supports.
• Ensure safety plans and legal documentation before placement.
• Provide ongoing training and support to foster parents and children.
• Deliver services across in-home and community settings; travel as needed.
