Job summary

Necco is seeking a Foster Care Case Manager to support children and foster families across office, school, community, and home settings. You will coordinate plans, services, and trainings to help children achieve permanency and well-being, while ensuring regulatory compliance and strong collaboration with partners.

You will manage treatment plans, safety assessments, and data entry, and travel between locations as essential.

Qualifications

• 21 years of age or older.
• Valid driver's license.
• LSW or LPC or equivalent degree in human services.
• Bachelor's degree with field experience of 2 years (WV) or higher degrees.
• Experience with behavioral health, family services, or foster care settings.
• Ability to navigate software (EHR, databases, dashboards) and meet deadlines.

Responsibilities

• Match referrals for children and families and advocate for best home match.
• Develop Initial and Comprehensive Treatment Plans for children.
• Coordinate assessments: biopsychosocial, independent living, etc.
• Refer to behavioral health, independent living and other services.
• Ensure legal documentation and safety plans prior to placement.
• Develop and monitor Individualized Treatment Plans (ITP).
• Provide training and support to foster parents and children.
• Maintain confidentiality and comply with state/licensure rules.
• Deliver services across in-home and community settings and travel as needed.
• Document and report incidents and participate in CAP when required.
