Job summary

Necco seeks a Foster Care Case Manager to partner with children and foster families across office, school, and home settings. You will help children reach goals, coordinate services, and ensure growth and permanency while complying with state licensing requirements.

You will develop and monitor treatment plans, facilitate assessments, and serve as liaison to social workers, schools, and community resources. Travel to support placements is expected.

Qualifications

• Must be at least 21 years old.
• Valid driver’s license and clean driving record.
• Degree in human services or equivalent with licensure requirements.
• Experience supporting children with emotional or behavioral needs is preferred.

Responsibilities

• Participate in referral matching and advocate for best home matches.
• Develop initial and comprehensive treatment plans for children.
• Oversee assessments and connect families to services such as behavioral health.
• Ensure legal documents are in place prior to placement.
• Create and monitor individualized treatment plans and CAP as needed.
• Serve as liaison with placement agencies and community resources.
• Attend service meetings and coordinate with schools and clinicians.
• Provide training and support to foster parents and children.
• Maintain confidentiality and document accurately in EHR.
• Travel to meet clients and transport as essential duties.
