Job summary

OVP Health is seeking a Foster Care Case Manager / Adoption Specialist to support children and foster families across office, school, and community settings. You will help achieve permanency through individualized service plans, assessments, and coordination with DHS and adoptions teams.

This role requires collaboration with schools, health and social service organizations, and ongoing training to ensure compliance with state licensure and agency standards.

Qualifications

• Bachelor's or Master's degree in social work or related field or bachelor's with department-approved training
• At least one year case management experience
• 21 years of age; Valid Driver’s License; willingness to travel; ability to meet background checks as required

Responsibilities

• Participate in referral matching for children and families; advocate for best home match
• Develop and monitor Individualized Service Plans and plan reviews
• Coordinate assessments (Biopsychosocial, CANS, Independent Living, Diagnostic, etc.) and service delivery
• Provide transportation and act as liaison with placement agencies and community resources
• Attend service meetings with schools, caregivers, and other stakeholders
• Ensure compliance with state regulations, confidentiality, and quality improvement processes

Education

Bachelor's or Master’s degree in social work or related field; or a bachelor's degree with department-approved training
