Job summary

One Family Illinois is seeking a Case Management Assistant to support the foster care team by coordinating visits, managing documentation, and communicating with families and service providers. This role emphasizes confidentiality, trauma-informed understanding, and collaboration with staff and community partners.

The position requires strong organizational skills, flexibility to work evenings/weekends, and reliable transportation for community travel. Training and supervision are provided.

Qualifications

• High School Diploma or equivalent; 1-2 years of college preferred.
• Experience with youth in educational settings preferred.
• Ability to pass DCFS criminal background check including CANTS/SACWIS and SORS.
• Valid Illinois driver's license with reliable transportation; travel within communities.

Responsibilities

• Schedules meetings and visits with youth and family.
• Obtains required reports and case documents from families and service providers.
• Receives and responds to routine requests for information and documents.
• Responds to routine inquiries from youth, family and the public regarding case information, services, and programs.
• Takes notes at child-family team meetings and hearings and follows up with case managers and youth/family.
• Maintains youth and family files and coordinates case information.
• Communicates with agency staff to coordinate case information.
• Attend supervision and participate in training and CQI initiatives.
