Job summary

One Hope United is seeking a Foster Care Case Manager in Illinois to engage diverse families and coordinate services across medical, educational, and mental health providers. The role requires strong communication, the ability to multi-task, and compliance with background checks.

You will conduct visits, develop plans, and coordinate placements with families while adhering to state and federal regulations. On-call duties may apply and computer skills are essential.

Qualifications

• Bachelor's degree required in a human services field or related area.
• Must have a valid driver's license and be able to pass background checks.
• Proficiency with computer basics and ability to work under pressure.

Responsibilities

• Serve as conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers.
• Conduct visits with children, parents and/or caregivers to assess safety, permanency and well-being.
• Develop and implement plans meeting the needs of the children and family.
• Facilitate placement and delivery of services with biological, foster and adoptive families.
• Perform case management in accordance with laws, codes, and policies within required timeframes.
• Attend court hearings and file all court documents within required timeframes.
