Job summary

Home Health Care, Inc. seeks an RN Visit Nurse to perform skilled nursing visits in patients’ homes across St. Paul and surrounding communities.

The role includes wound care, medication management, injections, and education, with travel throughout the area. The ideal candidate holds an active Minnesota RN license, has home health or related clinical experience, and can work independently while coordinating with a multidisciplinary team.

Qualifications

• Active Minnesota RN license required.
• Previous nursing experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment.
• Strong assessment and critical-thinking abilities.
• Confidence working independently in the field.
• Excellent patient and caregiver communication skills.
• Strong organization and time-management abilities.
• Ability to manage a daily schedule of patient visits.
• Reliable transportation and willingness to travel throughout the St. Paul service area.

Responsibilities

• Travel throughout St. Paul and surrounding communities providing skilled nursing care in patients' homes.
• Perform wound care and wound assessment.
• Medication reconciliation and management.
• Injections and other skilled nursing procedures.
• Monitor acute and chronic medical conditions; educate patients and families.
• Coordinate with physicians and other healthcare providers; collaborate with PT, OT, SLP, social work, and interdisciplinary team members.
• Complete timely documentation in Homecare Homebase (HCHB).
