Advocate Aurora Health, Inc. is seeking a physical therapist to join our float outpatient team. You will evaluate patients, develop and implement plans of care, and monitor outcomes with regular physician updates.

The role emphasizes mobility, safety, and patient education across clinics within a 45-minute travel radius. We offer a generous sign-on bonus, a $5/hour float premium, and a comprehensive benefits package with flexible scheduling.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• License to practice as Physical Therapist in Wisconsin or Illinois.
• BLS certification within 6 months of hire.
• Excellent communication and interpersonal skills.

Responsibilities

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, or integumentary evaluations.
• Establish a plan of care and implement treatment based on evaluation results and goals.
• Monitor outcomes and adjust treatment; provide status updates to physicians.
• Plan for and complete patient discharge with home programs and teaching.
• Complete documentation per department policy including progress notes and discharge summaries.
• Maintain clean, organized work area and proper equipment handling.
• Demonstrate knowledge of modalities and use of exercise equipment.
• Participate in performance improvement activities and staff orientation.
• Assist with coverage as needed and contribute to team goals.
