Job summary

Advocate Health is seeking a Physical Therapist to join our outpatient float team across Wisconsin and Illinois locations. You will evaluate musculoskeletal, neuromuscular, cardiopulmonary, and integumentary systems and develop individualized plans of care to meet patient goals.

This full-time role offers a sign-on bonus, a competitive pay range of $41.10–$61.65 per hour, comprehensive benefits, and flexible scheduling.

Qualifications

• Bachelor's Degree in Physical Therapy is required.
• No experience is required.
• BLS for Healthcare Providers within 6 months; State Physical Therapist license (WI or IL).
• Certification Addendum and hospital certification maintenance may be required.

Responsibilities

• Interprets referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluation and goals.
• Monitors outcomes and adjusts treatment and reports to physicians as needed.
• Plans for discharge and provides home programs and family teaching.
• Completes documentation including progress notes, discharge summaries, and regulatory records.
