Job summary

Advocate Health is seeking a Physical Therapist to join ourfloat team at outpatient clinics across Wisconsin/Illinois. You will evaluate patients, create and adjust treatment plans, and educate patients and their families to support ongoing recovery.

Applicants must hold a Bachelor's in Physical Therapy and a Wisconsin or Illinois PT license; BLS must be obtained within 6 months. The role offers a comprehensive benefits package and sign-on opportunities.

Qualifications

• Bachelor's Degree in Physical Therapy.
• BLS certification within 6 months (if required).
• Physical Therapist license for Wisconsin or Illinois.

Responsibilities

• Interpret referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care and implement individual treatment based on evaluation results and goals.
• Monitor outcomes and adjust treatment plans; inform physicians of progress.
• Discharge planning with home programs and patient/family teaching.
