Job summary

Advocate Aurora Health in Wisconsin is seeking a Physical Therapist to join our float team serving outpatient clinics within 45 miles of your home site. This full-time role includes training and resources to deliver care, with a 40-hour work week and a comprehensive benefits package.

Responsibilities include evaluating referrals, planning and implementing patient care, documenting progress, and ensuring safe discharge planning.

Qualifications

• Bachelor's Degree in Physical Therapy.
• BLS for Healthcare Providers certification within 6 months.
• Physical Therapist license issued in Wisconsin or Illinois.
• Must maintain annual hospital-based certifications as required.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluation results.
• Monitors outcomes and adjusts treatment plans; provides updates to physicians.
• Discharges patients with written home programs, teaching, and referrals.
• Completes documentation per department policy including progress notes and scheduling.
