Advocate Health is seeking a Float Therapist to provide care across outpatient clinics within a 45-minute radius. The role includes evaluating patients, creating treatment plans, and ensuring safe discharge with home programs.

This position offers a significant sign-on bonus and a $5/hour float premium, plus a comprehensive benefits plan and flexible hours. Applicants should have a Physical Therapist license in WI or IL, BLS, and excellent communication skills.

Qualifikationen

• Bachelor's Degree in Physical Therapy.
• BLS certification within 6 months (if required).
• Physical Therapist license in Wisconsin or Illinois.

Aufgaben

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluation results and goals.
• Monitors outcomes and adjusts the treatment plan based on patient response and reports to physicians.
• Discharges patients with home programs and teaching materials.
• Documents patient progress, billing, scheduling, and follows insurance guidelines.
• Keeps the treatment area clean and handles equipment, with safe lifting and transferring.
• Uses modalities and equipment appropriately with knowledge of indications/contraindications.
• Participates in performance improvement and caseload management; supports coverage as needed.
• Pursues professional growth and orients new staff; participates in evaluations.
• Delivers age-appropriate care and applies growth and development knowledge.
