Advocate Health in Illinois is seeking a float Physical Therapist for outpatient settings within 40 minutes of Advocate Sherman. The role is full-time, 40 hours per week, with a $5.00 per hour differential and a $3.00 weekend differential.

A $5,000 sign-on bonus is offered, payable in two installments after 90 days and after one year. The position requires interpreting referrals, developing and adjusting treatment plans, and ensuring thorough documentation while maintaining productivity

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Physical Therapy License in IL or WI, required to work.
• BLS for Healthcare Providers per AHA.
• No experience required for this role.

Responsabilités

• Interpret physician referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, or integumentary evaluations.
• Create and implement a care plan; monitor outcomes and adjust treatment.
• Plan discharge with home programs, education, and referrals.
• Document evaluations, progress notes, and discharge summaries per policy.
