Job summary

Advocate Health is seeking a Physical Therapist for a float position serving outpatient areas within 40 minutes of Advocate Sherman. The role is full-time, 40 hours per week, with a pay range based on experience and a $5.00 per hour differential plus a $3.00 weekend differential.

Requires Doctorate in Physical Therapy and IL or WI licensure. Must manage caseload, documentation, and collaboration with care teams to ensure optimal patient outcomes.

Qualifications

• Doctorate degree in Physical Therapy from an accredited program.
• Licensed in Illinois or Wisconsin to practice Physical Therapy.

Responsibilities

• Interprets referrals and conducts musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Establishes and implements a plan of care; monitors outcomes; adjusts treatment as needed.
• Discharges patients with home programs and teaching; coordinates with care team.
