Job summary

Advocate Health is seeking a full-time Physical Therapist to float to outpatient areas within 40 minutes of Advocate Sherman. The role includes a $5/hour float differential and a $3/hour weekend differential, plus a $5,000 sign-on bonus paid in two installments. Pay range is $41.10 to $61.65 per hour with standard benefits.

The therapist will interpret referrals, develop plans of care, document progress, and coordinate with physicians and team members to ensure optimal patient outcomes.

Qualifications

• Licensed in IL or WI; BLS for Healthcare Providers (AHA) within required period.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment; monitors outcomes and adjusts as needed.
• Discharge planning with home programs and patient/family teaching; coordinates referrals and equipment as indicated.
• Document all care per department policy; manage daily statistics, billing, and scheduling; adhere to policies.
