Qualifications

• Bachelor’s degree in Social Work or related field is required.
• Minimum of two years of case planning experience.
• Ability to travel between Staten Island and Brooklyn locations.
• Flexibility to work varied hours based on program needs.

Responsibilities

• Provide case coverage and support for EFFC cases as directed.
• Establish timely contact with children, families, and providers.
• Manage time-sensitive tasks and service coordination duties.
• Develop, implement, and monitor individualized service plans.
• Attend FTCs, FASPs, and court proceedings with documentation.
