Job summary

Seamen’s Society for Children and Families is seeking a Floating Case Planner to promote safety, permanency, and well-being for children in foster care. You will provide immediate case coverage and lead service planning with families, collaborating with providers across Staten Island and Brooklyn.

You will develop, implement, and monitor individualized service plans, complete timely documentation in CONNECTIONS, and participate in Court proceedings and family conferences.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• Minimum of two years of case planning experience required.
• Proficiency with Microsoft 365 apps and CONNECTIONS.
• Valid driver’s license preferred; ability to travel between Staten Island and Brooklyn locations.

Responsibilities

• Provide case coverage and support for assigned cases within the Enhanced Family Foster Care (EFFC) program.
• Review cases promptly to assess status and initiate case planning activities.
• Establish contact with children, families, foster parents, and service providers.
• Manage time-sensitive tasks and follow-up activities to maintain case oversight.
• Engage families in service planning and timely contacts.
• Monitor child safety, well-being, and permanency through regular contacts and home visits.
• Collaborate with teams to support medical, mental health, education, housing, and vocational needs.
• Maintain documentation in CONNECTIONS per agency standards.
• Attend FTCs, FASP meetings, and other planning activities within timelines.
• Attend Family Court proceedings and provide reports/testimony as needed.
• Prepare court reports and permanency documentation.
