Job summary

Seamen’s Society for Children and Families is seeking a Floating Case Planner to promote child safety, permanency, and well-being by providing case coverage across Staten Island and Brooklyn locations. You will coordinate service planning and collaborate with families, foster parents, and providers to develop, implement, and monitor individualized plans.

The role requires two years of case planning experience, a Social Work-related degree, and willingness to travel.

Qualifications

• Bachelor’s degree in Social Work or related field is required.
• Minimum of two years of case planning experience required.
• Valid driver’s license preferred.
• Ability to travel between Staten Island and Brooklyn locations as needed.

Responsibilities

• Provide case coverage and support for assigned cases within the EFFC program as directed by leadership.
• Review cases promptly to assess status, needs, and initiate case planning activities.
• Establish timely contact with children, families, foster parents, and service providers to ensure continuity of services.
• Maintain and document case activities in CONNECTIONS with accuracy and timeliness.
• Attend FTCs, FASP meetings, and other service-planning activities within established timelines.
