Job summary

Heartshare Human Services Of New York is seeking a Floating Case Planner to support case planners in the Division with day-to-day social services tasks in Brooklyn, NY. The role involves assisting children in foster care, connecting families to services, and aiding reunification efforts.

Under the Division Director, you will fill in for vacant positions as needed, participate in family visits and court-related activities, and collaborate with medical, mental health, and case management teams to

Qualifications

• Bachelor's degree or equivalent required.
• Knowledge of Child Welfare System preferred.
• Excellent written, oral, organizational, and interpersonal communication skills.

Responsibilities

• Assist case planners with day-to-day social services tasks.
• Support services to children in foster care to improve functioning.
• Connect parents to services and assist with family reunification.
• Provide coverage for visits, school activities, and safety contacts as needed.
• Attend Family Court hearings and participate in Family Team Conferences.
