Job summary

Trustbridge Hospice, part of Empath Health, seeks a compassionate Field-Based Emergent Care RN to provide urgent hospice visits across Palm Beach County. This role offers autonomy and meaningful patient-centered care in home and community settings.

You will triage and manage emergent conditions, collaborate with physicians and the interdisciplinary team, and educate families to support comfort and dignity during advanced illness.

Qualifications

• Florida-licensed RN required.
• Current Basic Life Support (BLS) Certification.
• 1 year in public health, home nursing, oncology, medical/surgical, geriatric, or hospice nursing preferred.
• Understanding of hospice nursing: case management, crisis intervention, communication, teaching, and thanatology.
• Basic computer skills and EMR experience a plus.
• Adequate and reliable means of transportation with valid Florida driver’s license.

Responsibilities

• Assess, triage, and manage urgent/emergent conditions in home/community settings.
• Operate with autonomy while collaborating with physicians and interdisciplinary teams to deliver timely, high-quality care.
• Advocate for patients and families, ensuring needs are addressed.
• Coordinate comprehensive care under the primary physician guidance with the team.
• Monitor patient conditions, provide interventions, and educate to enhance comfort and quality of life.

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide emergent hospice field visits throughout Palm Beach County. If you are driven to deliver meaningful, patient-centered care during critical moments, this role offers the opportunity to truly change lives—one visit at a time. You will work the following schedule: 6:00am - 4:30pm and every other weekend

What You\'ll Do

The Field-Based Emergent Care Registered Nurse (RN) provides rapid-response, patient-centered clinical care to individuals in home and community settings. You will:

• Be responsible for assessing, triaging, and managing urgent and emergent health conditions outside of traditional clinical environments.
• You will operate with a high level of autonomy while collaborating with physicians, advanced practice providers, and interdisciplinary teams to deliver timely, high-quality care and prevent unnecessary emergency department visits or hospitalizations.
• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life

What You\'ll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
