Job summary

The Salvation Army North and Central Illinois Division is seeking a SSVF Case Manager in Champaign, IL to support veterans and their families with housing stability and permanent housing solutions. You will conduct comprehensive assessments, develop individualized service plans, and coordinate with community partners to provide essential services and benefits.

This full-time, on-site role requires CTS/CS expertise, a compassionate approach, and the ability to travel within the service area.

Qualifications

Bachelor's degree in human services, social work, or related field.
One year of direct case management with homeless populations or Veterans.
Valid driver's license/MVR clearance.
Ability to work days, evenings, weekends, and holidays.

Responsibilities

Case Management (70%): Maintain program criteria, referrals and procedures for SSVF participants.
Provide intensive case management using CTI, Housing First, and Harm Reduction principles.
Develop individualized service plans within one week of enrollment with measurable goals.
Maintain contact with Veterans via visits, calls, and community meetings.
Conduct assessments to identify barriers to housing stability and related needs.
Assist Veterans in locating and maintaining permanent housing and conducting inspections.
Conduct intake and enrollment activities for eligible Veterans.
