Solomon Medical Inc. is seeking a full-time physician to support SNFs in San Jose, CA and Mountain View, CA. This role offers intermittent post-acute care coverage with a flexible schedule and potential part-time options.

The physician will work with a Nurse Practitioner, oversee care plans, and ensure compliance with Medicare and facility protocols. Vaccination is required.

Qualifications

• BC/BE primary care specialty, DEA, DHEC.
• California state license.
• Comprehensive knowledge of acute and long-term care treatment modalities.
• Chronic care disease states knowledge is required.

Responsibilities

Job description

Our healthcare organization is currently searching for a full time physician to assist at a San Jose, CA-based SNF and/or a Mountain View, CA-based SNF. The clinician will provide intermittent coverage for post-acute rehab patients and long-term residents.

We are looking for a physician who can monitor, deliver, and improve upon comprehensive quality patient care in an LTC Skilled Nursing, Assisted Living and Rehab setting. This is a full-time position (Monday through Friday only) but we’re open to part-time employment for the right candidate. There is great flexibility with this position – you make your own schedule.

This successful applicant will have the opportunity to apply a team concept approach to post-acute care delivery. The Physician will work independently, as well as, directly with a Nurse Practitioner to deliver and coordinate comprehensive care plans.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provide Medical Management and oversite of acute and long-term treatment plans
• Provide direct patient care for new admission, routine follow-ups including scheduled visits outlined by standard Medicare guidelines and Solomon Medical
• Receive support from a Nurse Practitioner and Medical Coordinator
• Drive compliance for all state and local regulatory requirements set forth by the facility
• Complete competency and advanced care plan on all new patients in the facility
• Daily documentation and billing with appropriate E/M coding based on Solomon Medical guidelines
• Coordinate treatment plans with ancillary staff at the facility
• Work directly with the President/Lead Medical Director to ensure facility protocols and care models are being met

QUALIFICATIONS:

• BC/BE primary care specialty, DEA, DHEC
• California state license
• Comprehensive knowledge of acute and long-term care treatment modalities
• Vast understanding of chronic care disease states REQUIRED
• Team player, efficient, detail oriented, organized and compassionate
• High energy and strong interpersonal communication skills is a must

• Prior experience in documentation within an EMR and billing/coding system

Employment is with Solomon Medical Inc., a post-acute care company that provides physician and NP services to SNFs across Northern California. This is a physician-led organization with an unwavering commitment to excellent patient care. The team is led by a well-respected physician, Dr. Matthew Joel Awerbuck, and sub-specialists. Pay is $175 per hour plus director fees and bonus.

NOTE: Full COVID-19 vaccination is an essential requirement of this role. Solomon Medical adheres to all federal, state and local regulations as well as all client/patient requirements and will obtain necessary proof of vaccination prior to employment to ensure compliance.
