Solomon Medical Inc. is seeking a physician to provide intermittent and potentially full-time coverage in a Gilroy, California skilled nursing facility.

The role involves direct patient care, coordination with a Nurse Practitioner, and adherence to Medicare guidelines within long-term and post-acute rehab settings. The position offers flexible scheduling, with emphasis on team-based post-acute care delivery, billing responsibilities, and leadership in care planning.

Qualifications

BC/BE primary care with active DEA license
California state license
Comprehensive knowledge of acute and long-term care treatment modalities
Experience with EMR and billing/coding systems

Responsibilities

Provide Medical Management and oversight of acute and long-term treatment plans
Direct patient care for new admissions and routine follow-ups per Medicare guidelines
Coordinate care with a Nurse Practitioner and Medical Coordinator
Ensure compliance with state and local regulatory requirements
Complete competency and advanced care plans on new patients
Daily documentation and billing with appropriate E/M coding
Coordinate treatment plans with facility staff
Collaborate with Medical Director to align care models
