Job summary

Rehab Without Walls Neuro Rehabilitation, an affiliate of BrightSpring Health Services, seeks a licensed Physical Therapist to deliver home- and community-based rehabilitation. You will conduct evaluations and design plans that extend beyond clinic walls into real-life settings like kitchens, grocery stores, and workplaces.

You will work autonomously within a collaborative therapy team, reassessing progress, providing patient education, and coordinating with physicians and families to optimise

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current BLS or CPR certification as per state requirements.
• Minimum of one year PT experience preferred.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and attend team meetings.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate needs and progress to the treatment team, patients, families.
• Coordinate with other disciplines to improve patient outcomes.
