Rehab Without Walls Neuro Rehabilitation is seeking an experienced Physical Therapist to provide functional rehabilitation in home, school, work, and community settings. You will assess clients, create and adjust treatment plans, and drive progress in real-life environments that extend beyond traditional clinical settings.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• CPR/BLS certification as required by state regulations.
• At least 1 year of work experience as a physical therapist preferred.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness; attend staffing meetings and family conferences.
• Conduct discharge evaluations; recommend home and community safety resources.
• Communicate patient needs and progress to the treatment team, physician, clients, and families.
• Collaborate with other disciplines to coordinate care and improve outcomes.
