Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a licensed Physical Therapist to provide functional rehabilitation across home, school, work, and community settings. You will evaluate clients, develop and adjust treatment plans, and support clients in real-life activities such as cooking, grocery shopping, dining out, and workplace tasks.

Qualifications

• Minimum of bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support (BLS) or CPR certification as required by state regulations.
• At least 1 year of PT experience preferred.
• Demonstrates knowledge of rehabilitation techniques for complex neurological injury.
• Good communication in verbal and written form.
• Ability to lift 50 pounds; may lift up to 100 pounds with safety precautions.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess treatment effectiveness; attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations and resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to coordinate care and improve outcomes.
