Qualifications

• Bachelor’s degree with relevant field; NC Medicaid-certified training within 90 days.
• Experience in case management, homecare, long-term care, or personal care preferred.
• Ability to navigate Medicaid programs and regulatory guidelines.
• Capable of maintaining detailed records and reporting as required.

Responsibilities

• Perform initial assessments and develop person-centered care plans.
• Coordinate services and referrals to community resources and providers.
• Monitor services through regular contact and home visits; document progress.
• Provide counseling and support to beneficiaries and families.
• Ensure documentation, billing codes, and compliance with regulations.
