RHA Health Services, LLC is hiring a CAP Case Manager to join our team in Buncombe & Madison counties, North Carolina. The PT, regular position supports children with complex needs and coordinates care with registered nurses and other providers.

Qualifications include a Bachelor's degree in social work or a related human services field, and relevant experience in case management, assessment, and referral. Benefits include Payactiv and health coverage for full-time staff.

Qualificações

• Bachelor’s degree in social work from an accredited school, or a related human services field with relevant experience.
• Directly related experience includes case management, assessment, referral, counseling, and treatment planning.

Responsabilidades

• Provide basic CAP Case Management services to medically fragile children in home settings.
• Coordinate care with RN to ensure quality, safe services and appropriate supports.
• Participate in pre-employment screening and credential verification as required.
• Develop person-centered plans and coordinate waiver services and educational support.
